Sigmoid volvulus vs Cecal volvulus
How to tell Sigmoid volvulus and Cecal volvulus apart when a board question makes them look alike.

Initial management branch point is endoscopic detorsion candidacy, which applies to stable sigmoid volvulus but not cecal volvulus.
Side by side
Older or chronically constipated patient with massive colonic distension and coffee-bean loop from pelvis toward right upper abdomen suggests sigmoid volvulus.
Younger patient with mobile cecum and acute obstruction showing dilated cecum displaced to left upper abdomen suggests cecal volvulus.
Redundant sigmoid colon twists around its mesentery causing closed-loop large-bowel obstruction and ischemia risk.
Congenitally mobile cecum rotates with terminal ileum causing closed-loop obstruction that is less amenable to endoscopic detorsion.
Abdominal x-ray can show inverted U or coffee-bean sign with apex toward right upper quadrant; CT may show mesenteric whirl and no distal rectal gas.
CT often shows markedly dilated cecum above 10 cm displaced to upper or left abdomen with whirl sign and small-bowel obstruction features.
The trap
Seeing the word volvulus and sending every patient straight to colonoscopy misses cecal cases that need urgent surgery; conversely, skipping decompression in stable sigmoid volvulus can increase ischemia risk while waiting.
The repair move
Next best step: if sigmoid volvulus is suspected and there is no peritonitis or perforation, perform urgent flexible sigmoidoscopic detorsion with rectal tube placement, then schedule definitive sigmoid colectomy during same admission to prevent recurrence. If cecal volvulus is identified or ischemia/peritonitis is present, proceed directly to surgery, typically right hemicolectomy with primary anastomosis when feasible.

CoreA 70-year-old man has abdominal distension, obstipation, and cramping pain. BP 114/70 mm Hg, HR 102/min, temp 37.2 C. X-ray shows a large coffee-bean loop arising from the pelvis, and he has no guarding or rebound. What is the next best step in management?
Show answer and explanation
Answer: A. Urgent flexible sigmoidoscopic detorsion with rectal tube placement. Stable sigmoid volvulus without peritonitis is initially managed with urgent endoscopic decompression.
Why the other choices are tempting
- B. Wrong because this operation is typically for cecal volvulus or ischemic bowel scenarios.
- C. Wrong because large-bowel closed-loop obstruction requires urgent intervention.
- D. Wrong because screening does not address acute obstruction.
- E. Wrong because antibiotics alone do not relieve mechanical torsion.

What management difference most quickly separates sigmoid from cecal volvulus?
Stable sigmoid volvulus is first decompressed endoscopically, while cecal volvulus generally requires urgent operative management.
Which radiographic direction of a coffee-bean loop supports sigmoid volvulus?
Apex projecting toward the right upper abdomen from a pelvic origin supports sigmoid volvulus.
Quick answers
What is the fastest way to tell Sigmoid volvulus from Cecal volvulus?
Initial management branch point is endoscopic detorsion candidacy, which applies to stable sigmoid volvulus but not cecal volvulus.
What is the key clue for Sigmoid volvulus?
Older or chronically constipated patient with massive colonic distension and coffee-bean loop from pelvis toward right upper abdomen suggests sigmoid volvulus.
What is the key clue for Cecal volvulus?
Younger patient with mobile cecum and acute obstruction showing dilated cecum displaced to left upper abdomen suggests cecal volvulus.
What is the common trap with Sigmoid volvulus vs Cecal volvulus?
